The neighbor says your truck sprayed in the wind. The inspector wants that day's record — product, EPA number, rate, wind speed and temperature at the time of application. It is in a spiral notebook, in a truck that is out on a route.

What Ohio Admin. Code 901:5-11-10 actually says
✓ Required
A record of each application — the applicator, the person contracting the service, the date, the target pest, the location and method of treatment, the product trade name and EPA registration number, the concentration or amount applied, the equipment, the time of day you started and finished, and wind and temperature. The employer retains those records for three years from the date of application.
✕ Not required
A particular form. The rule sets out what has to be recorded, not how — there is no state form, no prescribed layout, and nothing that says it has to be on paper. The spiral notebook in your truck cab is a habit, not a requirement.
