OSHA 29 CFR 1910.157(e)

The fire marshal is standing at the extinguisher holding a blank tag.

Somebody initialed twelve months down the tag in one sitting — same pen, same slant, same afternoon. Half the units in the back warehouse lost their tags in 2024. Nobody can say which ones are due for the six-year teardown.

What OSHA 29 CFR 1910.157(e) actually says

Portable extinguishers are visually inspected monthly and get an annual maintenance check. The employer records the annual maintenance date and keeps that record for one year after the last entry or the life of the shell, whichever is less.
✕ Not required
OSHA does not require a written record of the monthly visual inspection. The only record 1910.157(e)(3) names is the annual maintenance date. The punched tag is a convention — and where your local fire code adopts NFPA 10, the recording expectation comes from the fire marshal, not from OSHA. Ask yours which one they enforce.
