You tested nine devices on Tuesday. The carbons rode around in the truck until Friday. Two came back illegible, one had last year's gauge serial on it, and the water authority wants all nine resubmitted before it will clear the account.

Each backflow preventer is inspected and tested at least once every twelve months by a person the supplier of water approves, at the consumer's expense. Records of those inspections, tests, repairs and overhauls are kept by the consumer and made available to the supplier of water.
✕ Not required
The rule names no form. It does not mandate a state test sheet, a carbon-copy format, or one submission method — where it speaks to format at all it says paper or electronic. The form your water authority insists on is the authority's, not Ohio EPA's.
